How to Add a Customer Inquiry Form to a Wix Website Using Jotform
At HLN Solutions LLC, we wanted a simple, professional way for businesses to contact us about services, software solutions, and automation tools. Using Jotform's Wix integration, we created a branded inquiry form, connected it to our website, and successfully tested a real submission—all without custom programming.
Step 1 — Install Jotform on Wix
We opened the Wix App Market, selected Jotform: Form Builder & Payments, and added the app to our website. Next, we connected our existing Jotform account through the authorization process. This allowed us to create and display Jotform forms directly inside the Wix editor.
Step 2 — Create a Custom Inquiry Form
Using Jotform's AI Form Builder, we created a Small Business Service Inquiry Form with our company logo and blue-and-white styling. The form collects names, business information, email addresses, optional phone numbers, service interests, and messages.
Step 3 — Add the Form to Our Wix Website

Step 4 — Connect the Contact Page
We added a Submit a Business Inquiry button to our existing Contact page. Visitors can click the button to open the inquiry form in a new browser tab.
Step 5 — Test the Integration
We completed a test submission using sample information. The website displayed a customized thank-you message, and Jotform recorded the submission in its dashboard. This confirmed that the form worked from submission through receipt.
What We Learned
By integrating Jotform with Wix, HLN Solutions LLC created a professional customer inquiry system without custom programming. We can now collect business inquiries, review submissions in Jotform, and provide visitors with a convenient way to reach us.
